The modern technique most commonly used is the medial opening-wedge osteotomy, which is done by opening the bone on the inner side by the desired amount and fixing the bone with a locking plate to allow it to heal in the corrected position.

The aim is to spread the load. The mechanical axis of the leg passes through the diseased medial side in a bow-legged knee with medial compartment osteoarthritis. HTO shifts that axis toward the healthy outer compartment, taking the worn cartilage off the table. First the mechanics, then the pain relief. Remove the overload and the damaged compartment is protected and the symptoms settle.
